Unfortunatley, if the conditions of a spousal permit do require that the foreigner maintain a relationship with the South African citizen.
To extend you will need to present much of the same documentation you did the first time around. You will need to provide letters from your spouse, proof of joint income, etc.
I think it might be a good idea to enlist the services of an immigration lawyer, they will be able to give you a better idea of where you stand currently.
